Family Planning Waiver Program
The Family Planning Waiver Program provides contraceptive services and related reproductive health care services to eligible women in Wisconsin. Birth control methods, supplies and family planning health care services are included in the program. Click here for the Spanish version of this fact sheet. Who is Eligible? There are several requirements you must meet in order to qualify for the Family Planning Waiver Program. You might qualify if: • You are a woman between 15- 44 years old. • You are a legal U.S. resident living in Wisconsin. • You do not receive Wisconsin Medicaid, BadgerCare, or Healthy Start. • You meet these monthly income limits (valid through February 2007):
o One-person household $1,511 o Two-person household $2,035 o Three-person household $2599 o Four-person household $3,083 There’s a good chance that you qualify if you meet all of these requirements. Parental income is not factored into teens or college students’ eligibility for this program.What Services are Included? Routine reproductive health care visits, birth control supplies, pregnancy testing and counseling, emergency contraception, pap smears and cancer screening, sexually transmitted infection tests and treatment, and education and information including natural family planning are all included in the program. Where Can I Receive Services? Any Medicaid-certified provider can offer the Family Planning Waiver Program. You can receive services from family planning clinics, health departments, physicians, nurse practitioners, and federally qualified health centers. To Apply for These Free Services If you live in Adams, Junea, Langlade, Lincoln, Marathon, Portage or Taylor counties, call Family Planning Health Services at 1-800-246-5743 to enroll in the program. If you live elsewhere in the state, call Planned Parenthood of Wisconsin at 1-800-230-PLAN to find the health center nearest to you. When you are at a clinic, you may apply and receive same-day services. You may be covered temporarily (for up to three months) when you sign up to receive same-day services.
